BPE chart

BPE codes at a glance

The Basic Periodontal Examination scores each sextant from 0 to 4, with * for furcation involvement. Record the highest code found in each sextant.

CodeWhat you seeUsual next step
0No pockets over 3.5 mm, no calculus or overhangs and no bleeding on probing. No periodontal treatment is usually needed. Repeat the BPE at the routine recall.
1Bleeding on probing, with no pockets over 3.5 mm and no calculus or overhangs.Oral hygiene instruction.
2Calculus or plaque-retentive factors such as overhanging margins, with no pockets over 3.5 mm.Oral hygiene instruction and removal of calculus and plaque-retentive factors.
3Probing depth of 3.5 to 5.5 mm: the black band of the probe stays partly visible.Oral hygiene instruction and root surface instrumentation. Consider radiographs, and record a six-point pocket chart for the code 3 sextants after initial therapy.
4Probing depth over 5.5 mm: the black band disappears into the pocket.Oral hygiene instruction, root surface instrumentation, radiographs and a full six-point periodontal chart. Consider whether specialist referral is needed.
*Furcation involvement, recorded next to the code for that sextant.Assess the affected teeth in detail. Specialist referral may be indicated.

Six-point perio chart

What the periodontal chart records

Each tooth has three sites on the buccal side and three on the palatal or lingual side: distal, middle and mesial.

CEJRecession 2 mmProbing depth 5 mmAttachmentloss 7 mm
Probing depth
From the gingival margin to the base of the pocket. Sites of 4 mm or more are highlighted, and 6 mm or more stand out further.
Recession
From the cement-enamel junction to the gingival margin. Enter a negative number when the margin sits above the junction, for example with swelling.
Attachment loss
Probing depth plus recession at each site. The chart works it out for you and shows the most loss in the summary.
Bleeding on probing
Tick each site that bleeds. The summary shows bleeding as a share of the sites on the teeth you charted.

FAQ

Using the chart

Quick answers about the free chart, your data and the BPE guide.

Is this periodontal chart really free?

Yes. There is no login, no email and no limit on how often you use it.

Which BPE codes can I enter?

Codes 0 to 4 for each sextant, * for furcation involvement next to the code, and X for a sextant with fewer than two teeth.

How do I record a value of 10 mm or more?

Use the up and down arrow keys on a site to change its value, or switch off auto-advance and type both digits.

Does it work on a tablet or phone?

Yes, in any modern browser. On a phone the chart scrolls sideways, so a tablet or desktop is easier for a full chart.

FAQ

Privacy and saving

Is anything I enter stored or sent anywhere?

No. The chart runs in your browser on this device. We do not ask for patient names, and the scores are not sent to Medicasimple or to analytics tools.

How do I keep a copy?

Download the PDF or print the chart before you close the page. The printout has space to write the patient reference and clinician by hand.

Is the BPE guide a diagnosis?

No. It is a general guide based on the highest code entered. Use it alongside current BSP guidance and your own clinical judgement.
