Consent Form
Create a consent form template, prepare a form for a patient, share it and track its approval in one guide.

Consent forms are created as reusable templates in Settings, then prepared for an individual patient and treatment plan from the patient's Forms tab. This guide follows the current English interface from setup through approval.
Review your existing forms: Check the wording, clinic details and approval experience before using a new template with patients. Your clinic is responsible for making sure its final consent text meets its requirements.
1. Create a consent form template

Go to Settings → Forms and open Consent Form.
Click New Consent Form and enter a Form Header.
Optionally select a Form To Open After Approval, such as another form the patient should complete next.
Enter your clinic's consent wording in Form Content. The editor includes patient and clinic parameters, including patient name, identity number, phone number, doctor details and creation date.
If applicable, select Show treatment plan in form. Review the final text and click Create.

Templates can be prepared for different procedures where your clinic needs different wording. Once saved, a template is available for patient-specific forms, but it is not attached to treatment plans automatically.
Do not treat the default editor content as final legal wording. Adapt and check the text for your clinic before using it.
2. Create a form for a patient

Open the patient's profile and select the Forms tab.
Choose the Consent Form card and click New Consent Form.
Select the Form Template and the relevant Treatment Plan, then click Create.

Check that the plan's treatments and selected teeth or treatment areas are correct before preparing the form. Creating a reusable template does not by itself create a consent form for every treatment plan.
3. Share the form and follow its status
In the patient's consent form area, choose SMS, E-Mail or Open in New Tab, then select Send Consent Form where appropriate. Confirm the patient's contact details before sending. Until approval is completed, the form shows Pending and states that it has not been approved yet.

You can also use Print if the patient needs a paper copy. Have the patient sign it and follow your clinic's procedure for recording and storing the signed document in the patient record, for example under Media.
4. Check the approval record
Return to the patient's Forms → Consent Form area to check the status. After completion, the form shows Approved with the approval date and recorded approval details. Review the record rather than assuming that sending the form means consent has been given.

Permissions and good practice
Creating or editing templates requires the Form Management permission.
Check patient contact details before sending and review the resulting approval status rather than treating delivery as consent.
Review each procedure-specific template as clinical or legal requirements change. Ask your clinic's legal adviser to assess the suitability of the final wording.
