Patient-Specific Consent Form
How to create a consent form for a specific patient and share it with the patient for approval.

After preparing a reusable consent form template, you can create a patient-specific consent form linked to the relevant treatment plan and share it for approval.
1. Open the patient's Consent Form area
Open the patient's profile.
Select the Forms tab.
Choose the Consent Form card.
This area lists any consent forms already created for the patient and lets you add a new one.

2. Create a patient-specific consent form
Click New Consent Form.
Select the appropriate Form Template.
Select the Treatment Plan that the patient will approve.
Click Create.
Before creating the form, check that the correct teeth or treatment areas are selected for every treatment included in the plan.

3. Share the form and monitor approval
After the form is created, choose the delivery method that suits the patient:
SMS: send the consent form link to the patient's registered mobile number.
E-Mail: send the link to the patient's registered email address.
Open in New Tab: open the form immediately on the current device for an in-clinic review.
Select the preferred channel and click Send Consent Form. Until the patient completes the approval, the form displays a Pending status and the message that it has not been approved yet. The status is updated after the patient completes the form.

4. Collect a signature manually when needed
If the patient cannot complete the digital approval, click Print, have the patient sign the printed form, and upload a scan or clear photo to the patient's Media area for secure storage.
