Patient-Specific Consent Form

How to create a consent form for a specific patient and share it with the patient for approval.

After creating the consent form template in general, you can create a consent form for each patient based on a specific treatment plan.

When you open the Forms area from the patient's profile, no saved form will appear if you have not created one before. Click the New Consent Form button on the right. First, you will be asked to select the consent form template. After selecting the template, choose the treatment plan you want the form to be created for and click Create. A consent form will then be generated for the selected treatment. Before creating the form, remember to check that teeth have been selected for all treatments.

You can send the created form to the patient's registered phone number by SMS and receive the patient's approval. To do this, click the Send SMS button on the right. The patient will receive both the consent form link and a 6-digit verification code for approval.

When the patient opens the consent form link, enters the 6-digit code, and approves it, you will see a confirmation message on your screen as shown below.

If the patient has not approved the form yet, the same area will show that this form has not been approved.

This is how you collect consent approval from a patient by SMS. Some patients may not be able to approve a consent form by SMS. In these cases, you can click the Print button on the right, print the form, and ask the patient to sign it.

You can then take a photo of the signed consent form and add it to the patient's images for secure storage.
