Why It Matters

Consent forms play a critical role in both patient safety and legal compliance.
They ensure that patients are fully informed about the procedures they will undergo and that their explicit approval is documented.

With Medicasimple, clinics can create, manage, and share consent forms digitally; reducing paperwork, minimizing errors, and keeping all patient consents securely stored in one place.


What You Can Do

  • Create customized consent form templates for each treatment type.

  • Add and edit form content directly from your settings panel.

  • Automatically attach consent forms to specific treatment plans.

  • Send consent forms via SMS for digital approval or print for signature.

  • Track whether a patient has viewed, approved, or not yet confirmed the form.


How It Works

1. Creating a New Consent Form Template

  1. Go to Settings → Forms → Consent Forms.

  2. Click “New Consent Form.”

  3. Enter a Form Title (e.g., “Implant Procedure Consent”).

  4. Write or paste your consent form content into the text field.

  5. (Optional) Enable “Show Treatment Plan in Form” to display the patient’s treatment details inside the form.

  6. Click Create to save your new template.

✅ Once saved, the template will be available to use across all patient profiles and treatment plans.

2. Generating a Patient-Specific Consent Form

After creating your general template, you can generate a patient-specific form linked to a treatment plan:

  1. Go to the patient’s profile → Forms tab.

  2. Click “New Consent Form.”

  3. Select a template from your existing list.

  4. Choose which treatment plan this consent should be attached to.

  5. Click Create.

💡 Tip: Before creating the form, make sure the related treatment includes all selected teeth or areas.


3. Sending Consent Forms to Patients

Once the form is ready, you can send it digitally for approval:

  1. Click the “Send SMS” button on the right side.

  2. The patient will receive:

    • A secure link to view the consent form.

    • A 6-digit verification code for identity confirmation.

When the patient enters the code and confirms the form, the status in your system automatically updates to Approved.

If the patient hasn’t approved it yet, you’ll see “Pending approval” under that form.


4. Handling Manual Signatures

If the patient cannot complete the digital process:

  • Click “Print” to generate a physical copy.

  • Have the patient sign the printed form manually.

  • Take a photo of the signed document and upload it under Media for secure archiving.


5. Tracking Consent Status

You can view each form’s approval state directly in the patient’s Forms tab.
Possible statuses include:

  • Approved — patient completed SMS verification.

  • 🕓 Pending — form sent but not yet approved.

  • Not Sent — no action has been taken yet.

This helps your clinic maintain a full audit trail for every patient’s consent process.


Permissions & Recommendations

  • Only users with the Form Management permission can create or edit templates.

  • SMS delivery should always be tested using a non-Medicasimple account, since internal users are redirected to the homepage.

  • Always confirm that patient phone numbers are correct and active before sending digital consents.

  • Keep a printed backup for critical procedures where local regulations require physical signatures.


Best Practices

  • Create one template per major procedure type (e.g., Implant, Whitening, Extraction).

  • Regularly review consent text for updated legal or clinical requirements.

  • Use the digital SMS flow to save time and ensure verifiable patient identity.

  • Store all signed forms (digital or scanned) securely under each patient’s record.
